If your employer has registered your group policy with Lincoln, you can log in to access the policy. Otherwise, please confirm your coverage or eligibility directly through your employer.

How do I make a change to my plan?

Check with your employer to find out how often you can change or stop your contributions. You can also register with our online service site, www.Lincoln4Benefits.com , to take advantage of convenient services and find the answers you need.

You may submit your claim information to us through our online service site www.Lincoln4Benefits.com or by calling 800-423-2765. After we’ve received the required information, a claims examiner will review it and approve or deny the claim.

How to submit a claim

Please use the information below to submit a claim for an employee insurance product. Please note that state specific provisions are applicable.

For claims and benefit questions, please call 800-423-2765, or log in and access information online here. Registration is required.

  

Weekly Income/Short-term disability

You can submit claims by phone or by completing a claim form.

To submit a claim by phone, please call 866-783-2255.

Claim forms may be downloaded by going to  www.Lincoln4Benefits.com . Directions for submission are included on the claim form.

If you’ve already submitted a claim, you can log in 24/7 to check your claims status. Registration is required. 

Leave of absence (family medical leave with short-term disability)

You can submit claims by phone or by accessing the online portal.

To report an absence, including short-term disability, by phone, please call 800-423-2765.

Or you can log in  to the online portal and select Absence Management. Registration is required.

If you’ve already submitted a claim, you can log in  24/7 to check your claims status, report intermittent time or report a return to work.  Registration is required.

Long-term disability

You can submit claims by completing a claim form.

Group Life and AD&D

Beneficiaries can submit claims by phone or by completing a claim form.
