Electronic fund transfer of premium or loan payments
You can sign up for electronic funds transfer by logging in to your account online.

Sign up for EFT
Initiate the change to Electronic Funds Transfer (EFT) by completing the Authorization form. Print, fax, email or mail your completed form using the contact information listed on the form. (PDF)

Payor name and address changes
You may change the payor's name and address by:
- Reviewing your current information by logging in to your account online. Log in
- Initiating the change by printing and faxing, emailing or mailing the Life Address Change and/or the Life Name Change form found here. For contact information, please use the contact information on the form.

Helpful hints
Applying payments to your policy
You can help ensure your payment is applied promptly and accurately by including your policy number and noting “premium payment” or “loan payment” on your check.
Payment methods
Premium or loan payments may be made on a monthly basis through the Electronic Funds Transfer program. Payments may also be made through direct billing on a quarterly, semi-annual or annual basis. Please contact customer service for additional questions.
Requesting a duplicate premium notice
Payments may be made without the premium or loan notice. Be sure to include your policy number and note “premium payment” or “loan payment” on the check. If you would like a duplicate notice for your records, please contact customer service.
- Sending a payment via mail: To remit a payment via mail, please use the billing address found on your billing notice. If you would like to overnight a payment, please use the address for Customer Service for the appropriate product.
