Please use the information below to submit a claim for an employee insurance product. Please note that state specific provisions are applicable.
For claims and benefit questions, please call 800-423-2765, or log in and access information online. Registration is required.
You can submit claims by phone or by completing a claim form.

If your employer-sponsored plan is effective on or after 1/1/19 (policy series* GL501), you can submit a claim by emailing Fileclaim@lfg.com, or faxing (888) 735-7636. Claims may also be submitted electronically through the self-service portal. To access the portal, log in to your account at LincolnFinancial.com. You can file your Health Assessment Benefit claim telephonically by calling 800-423-2765. If you have questions about a claim, email claim@lfg.com or call 800-423-2765.

If your employer has registered your group policy with Lincoln, you can log in to access the policy. Otherwise, please confirm your coverage or eligibility directly through your employer.
Check with your employer to find out how often you can change or stop your contributions. You can also register with our online service site, www.Lincoln4Benefits.com , to take advantage of convenient services and find the answers you need.
Please complete our Lost policy inquiry form to locate a misplaced Lincoln policy, contract or account.
Life Claim
You can start a claim, review your coverage or download forms on our online service site www.Lincoln4Benefits.com . For additional claims and benefit information, please call 800-423-2765, or log in to your account. You can also register here.
Disability
You may submit your claim information to us through our online service site www.Lincoln4Benefits.com or by calling 800-423-2765. After we’ve received the required information, a claims examiner will review it and approve or deny the claim.
Evidence of Insurability
- Reach out to your HR Representative for your company code to complete your Evidence of Insurability application.
- Visit www.MyLincolnPortal.com
- Register as a new user with your employer's unique company code. If you have logged into MyLincolnPortal.com previously, then log in with your username and password.
- Select "Complete Evidence of Insurability" and follow the prompts to answer all required questions.
- Watch this short video to learn more!
- We'll process your application as quickly as possible, and if we need any other medical information from you, you'll receive a letter by mail.
- You'll be notified in writing once we approve or deny your application. You'll be able to see the status of your application at www.MyLincolnPortal.com in 24 hours.
- We'll also notify your employer of the final decision regarding your coverage. If your coverage is approved, you may see adjustments to your payroll deductions.
